THREE WEEKS PRIOR TO THE INDUCTION CEREMONY, FACULTY ADVISORS SHOULD SUBMIT THE FOLLOWING TO NATIONAL HEADQUARTERS:
- A typewritten LIST with each student’s name and a permanent (not school) mailing address (do not submit personal record cards)
- The DATE OF INITIATION (month, day, year)
- ONE CHECK payable to “Phi Alpha Theta” to cover the $40 per member induction fee (we cannot accept individual student checks)
